    題名: Trends in depressive symptoms and the determinants of changes among Taiwanese elderly: A cohort study between 1994-2004

    摘要: This longitudinal study was conducted between 1994 and 2004 based on a community-dwelling elderly cohort in southern Taiwan. This study aims to examine trends in depressive symptoms of elderly people and the determinants associated with the changes, using a ten-year longitu-dinal data with six waves of cross-sectional data. The Short Psychiatric Evaluation Schedule (SPES) was used for collecting information on depressive symptoms. Socio-demographic factors, socioeconomic fac-tors, living arrangements, health-related behaviors, chronic diseases, and disability were included as the predictors of depression symptoms. The significance of the differences between the trends of depressive symptoms was examined by using the generalized estimating equation (GEE) approach. The study showed great complexity in the develop-ment of depressive symptoms. It is worth noting that psychological com-ponents of depressive symptoms were more likely to increase over time. Being female, not married, living alone, with low socioeconomic sta-tus, negative life style, having chronic diseases, and ADL/IADL dis-abilities were significant predictors of depressive symptoms. This is the first study that has ever examined depressive symptoms in elderly pop-ulation over a ten-year time span and found a persistently increasing trend of depressive symptoms. The present findings confirm that depres-sive symptoms in ageing population are complex and characterized by individual variations in socioeconomic resources, physical health and functioning. It seems that disability is of central significance of late life depression. Effort to slow down the increasing trends of depression in the elderly should target the risk factors associated with chronic dis-eases, disability, and persistent depressive symptoms.
