Primary operation followed by adjuvant (chemo-)radiotherapy (OP-CRT) or definitive chemoradiation (CCRT) is the mainstay treatment for hypopharyngeal squamous cell carcinoma (HPSCC). Our study demonstrated that OP-CRT and CCRT had similar 3-year overall survival. However, compared with CCRT, OP-CRT exhibited significantly better local and regional controls, especially for HPSCC patients with T3, T4, or N2b/N2c disease, but was more likely to fail treatment due to subsequent distant metastases. We suggest that tailored management of distinct failure patterns by different primary therapies may improve the prognosis of HPSCC (https://doi.org/10.1245/s10434-022-12744-1).
